Job Description
As a Travel Registered Nurse (RN) specializing in Case Management, you will be responsible for coordinating and managing patient care across the continuum, ensuring that patients receive the appropriate care while optimizing resources and minimizing costs. You will work closely with patients, families, physicians, and healthcare teams to develop and implement individualized care plans, ensuring that patients are transitioned smoothly through various stages of care.
In this travel nurse role, you will take short-term assignments at healthcare facilities across the country, helping to manage patient cases, ensure continuity of care, and meet regulatory and insurance requirements. This opportunity allows you to apply your case management expertise while experiencing new healthcare environments and locations.
Key Responsibilities:
Coordinate Patient Care: Manage the coordination of care for patients from admission through discharge and post-discharge, ensuring that care plans meet patients' medical, psychosocial, and financial needs.
Develop Care Plans: Work with physicians, healthcare teams, patients, and families to create personalized care plans that ensure timely and appropriate care delivery.
Utilization Review: Conduct regular assessments of patient care to ensure that healthcare resources are used efficiently and appropriately, in line with insurance policies and hospital protocols.
Discharge Planning: Plan and implement safe and efficient discharge processes, including referrals to home health services, rehabilitation, or other post-acute care settings.
Patient Advocacy: Serve as a patient advocate, ensuring that patients and families understand their treatment options, available resources, and care pathways.
Collaboration with Healthcare Teams: Collaborate with multidisciplinary teams, including doctors, nurses, therapists, and social workers, to ensure that patient care plans are executed effectively.
Documentation and Compliance: Maintain accurate patient records, ensuring compliance with hospital policies, legal requirements, and insurance guidelines.
Adapt to Various Healthcare Settings: Quickly integrate into different hospital systems and case management processes as you transition between travel assignments.
Job Requirements
Education:
Active Registered Nurse (RN) license in good standing.
Bachelor of Science in Nursing (BSN) preferred but not required.
Experience:
Minimum of 2 years of recent experience in Case Management or related areas such as discharge planning or care coordination.
Experience in a hospital or acute care setting is preferred.
Certifications:
Case Management Certification (CCM) or equivalent preferred.
BLS (Basic Life Support) certification may be required by some employers.
Licensure:
Active, unrestricted RN license in the state of assignment or a compact/multi-state license.
Skills:
Strong organizational and multitasking abilities, with a focus on efficiency and attention to detail.
Ability to manage complex care plans and coordinate across multiple healthcare providers.
Excellent communication and interpersonal skills, particularly when working with patients, families, and healthcare teams.
Knowledge of utilization management, insurance, and healthcare reimbursement practices.
Proficient in documentation and healthcare compliance standards.
Flexibility and adaptability to work in various hospital environments and case management systems.
Travel Requirements:
Willingness to travel to different healthcare facilities for 8- to 13-week assignments.
Flexibility to work a variety of shifts, as needed, depending on the assignment.
